Psychological Therapeutic Consultation is a clinical service that addresses challenges and symptoms with the individual and their team to increase quality of life and support personalized goal achievement.

Payment Accepted
FIS (Family & Individual Supports) Waiver, CL (Community Living) Waiver, and FAPT (Family Assessment and Planning Team) funding. Private Pay available.
Hours of Support
Appointments take place within weekday business hours. Locations and frequency are determined by the individual and their support team, based on clinical availability.
Staffing
Services are provided by a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).
Why Wall for Psychological TC?
A rare waiver service in Virginia designed to address challenges and symptoms with an individual and their team to increase quality of life and support meaningful, customized goals. Wall Therapeutic Solutions became one of the first in the state to offer this service when Psychological Therapeutic Consultation began at Wall Therapeutic Solutions in 2023.
A Customized Approach
Unlike outpatient therapy, clinician involvement is not limited to an office setting or to the individual alone. This team-based approach allows the clinician to collaborate across environments to address factors affecting the person’s ability to reach their potential. Symptoms of diagnosed conditions are targeted rather than behaviors or actions, therefore data collection typically isn’t part of this service.
Assessment and Observation
- The clinician conducts a BioPsychoSocial-Spiritual Assessment through interviews with the individual and key people in their life to create a full picture of the person’s life and experiences to date. This helps identify relevant history, cultural factors, trauma considerations, preferences, and support needs. These conversations continue throughout the service to ensure plans evolve appropriately.
- The clinician also observes the individual in routine activities and interactions with others in their natural environment. This means that a psychological consultant literally meets a person where they are. This may include their home, school, workplace, day program, doctor’s office, or community settings.
- Assistive technology assessments may be included, addressing items such as phones, watches, alarms, calendars, adaptive tools, safety equipment, and health trackers. With consent, photos may be used to support self-esteem, communication, and social story development.
Goal Setting and Progress
- Goals to increase quality of life may be identified quickly or explored gradually with clinician guidance. Examples may include things such as reducing stress or making more friends. Plans evolve as goals are met or needs change.
- The clinician demonstrates strategies, provides training, and offers education on diagnoses, symptoms, and recommended interventions.
- Ongoing consultation about goals, outcomes, and progress or regression occur in person, by phone, or virtually. Communication is key for maximum problem-solving benefit. The clinician promotes consistency across team members so each voice is heard and the individual receives the most cohesive support possible.

Goals are identified by the individual and/or their support team. Common focus areas include:
- Adjusting to transitions, change, grief/loss, and addressing rigid thinking patterns
- Self-awareness and identity development
- Effective communication, self-confidence, and self-advocacy
- Healthy lifestyle habits
- Improving and/or increasing relationships
- Coping with symptoms of diagnosed conditions (paranoia, perseveration, memory challenges, self-stimulation, anger management, etc.)
- Safety and legal risk management
- Team strategies to overcome specific challenges
- Developing strengths, trying new activities, and exploring interests
Yes. These services are different and may occur concurrently. With permission the clinician may collaborate with co-treating professionals to ensure consistency across environments and inform plan development. It is up to the person and/or their authorized representative whether or not to give this permission.
No. Although Psychological Consultation can be delivered by psychologists or psychiatrists, Wall offers it through Licensed Clinical Social Workers (LCSW) and/or Licensed Professional Counselors (LPC), as available. No IQ testing is performed. Copies of past evaluations are requested, if available.
In-person service depends on clinician location. Telehealth often supplements in-person sessions when distances are significant. Monthly extended sessions may occur to maintain consistent rapport. Team members may still participate through telehealth even when the individual prefers not to.
The initial authorization is typically six months, followed by yearly authorizations as needed. It is a consultative service, not a long-term continuous support. Services may restart if new needs arise.
Scheduling is based on the individual and their support team’s preferences and tolerance, along with clinician availability. Some individuals prefer short weekly check-ins, while others participate less frequently but in varied settings with different team members.
No. Psychological Consultation is not a direct care service. Clinicians cannot administer medication administration or provide personal care. This service cannot duplicate other Medicaid services.
This is not a crisis or on-call service. Emergencies must be directed to crisis services or emergency responders. Non-urgent matters can be left via phone or email for follow-up.
Services require consent and may be discontinued at any time. Feedback is encouraged and reviewed at least quarterly with the individual or their authorized representative.
